LIABILITY RELEASE AGREEMENT
STATEMENT OF RISKS, ASSUMPTION OF RISKS AND RELEASE
I, THE UNDERSIGNED FOR MYSELF OR ON BEHALF OF ANY MINOR INDICATED BY ME BELOW, AGREE TO THE FOLLOWING: (please initial on each line)
1) I understand that Get Outside's activities may be physically and emotionally demanding.
2) I have voluntarily chosen to participate in Get Outside knowing that it brings risks of death, injury, damage or loss to myself and my property, from such events as falls, rockfall, avalanches, lightning, river crossings, hypothermia, frostbite, heat or cold injuries, bites and stings, and accidents traveling to and from the activity site. I understand that equipment may fail, and such failure could contribute to my injury or death. My participation in these activities (or that of the minor on whose behalf I sign this contract) is completely voluntary and is undertaken in spite of the hazards and risks involved.
3) I understand that I am solely responsible for assessing whether my skills, equipment, and level of physical and mental fitness are adequate for participation in program outings. I represent that I have full knowledge of the skills, equipment and level of physical and mental fitness necessary for participation on program outings and that I (or any minor on whose behalf I am signing) meet or exceed these requirements.
4) I understand that program outings involve activities in rugged terrain in all extremes of weather, and I may be very far removed from any professional or medical services. I understand that rescues may take more than 24 hours and all medical and rescue costs are my responsibility. I understand the importance of obtaining sufficient medical and accident insurance before participation in the activity.
5) I expressly accept all risks associated with this activity and voluntarily assume full responsibility for all the consequences of my choosing to participate in program outings. I hold Get Outside, and its members, coordinators, and trip participants, harmless for any death, injury, damage, or loss which I (or any minor on whose behalf I sign this contract) might suffer as a result of participation in club outings, including any injuries or damages which result from another's negligence or fault.
6) For myself and for any minor on whose behalf I sign this contract, I hereby waive and relinquish any claims or causes of action which may arise out of my activities as a participant in program outings, for myself and on behalf of my spouse, next of kin, members of my family, heirs, and for personal representative and my estate, and I agree that under no circumstances will I or my family, my heirs, executors, administrators or assigns present any claim for death, injury, damage, or loss against Get Outside or any of its members, coordinators, or trip participants, whether or not the claim arises due to any person's negligence or fault.
It is my intention to relieve every other person involved with Get Outside, and the organization itself, from any liability, whatsoever, which arises due to my participation in this program. I make this release and waiver of liability of Get Outside members, coordinators, and trip participants, even if they negligently or by some other act of omission cause me injury or death.
I HAVE READ AND UNDERSTOOD THIS ENTIRE DOCUMENT, AND AGREE THAT IT IS THE ENTIRE AND SOLE AGREEMENT BETWEEN MYSELF AND GET OUTSIDE CONCERNING THE TOPICS IT ADDRESSES. THIS AGREEMENT AND DOCUMENT HAS BEEN PRESENTED TO ME IN A WAY THAT ALLOWS ME TO REALISTICALLY CONSIDER ITS IMPLICATIONS AND TO CHOOSE NOT TO SIGN IT, AND TO DECIDE WHETHER OR NOT TO PARTICIPATE IN THIS PROGRAM.
In addition, parents must also sign a separate release form for each child under the age of 18:

As a parent or legal guardian of a minor under 18 years of age who will participate in the activities of Get Outside, I have read this contract and document and sign this release on his or her behalf. I understand and agree that every provision in this contract applies to the minor on whose behalf I sign this contract, just as it would apply to me if I were signing it for myself.
