Replacing Diplomas

To replace a lost/damaged diploma, submit a request in writing to:

The University of Tulsa
Office of Registration and Records
800 South Tucker Drive
Tulsa, Oklahoma 74104-9700

The request must include:

  • the reason for replacement
  • the old diploma must be returned
  • your signature

Only one diploma per degree will be issued for a student.

The cost of replacing a diploma is $25.00 and must be included with the request.
