Patient Rights and Responsibilities

GOAL

To provide patients with their rights and responsibilities while being treated at the Alexander Health Center.  All contacts at the AHC are strictly confidential as protected by HIPAA laws and professional ethics.

OBJECTIVES

  • To assure that all patients are treated with respect, consideration and dignity.
  • To provide appropriate privacy for patients.
  • To assure that all patient disclosures and records are treated confidentially, and, except when required by law, to assure that all patients are given the opportunity to approve or refuse their release.
  • To provide patients, to the degree known, complete information concerning their diagnosis, evaluation, treatment and prognosis. When medically inadvisable to give such information to a patient, the information will be provided to a person designated by the patient or to a legally authorized person.
  • To give patients the opportunity to participate in decisions involving their health care, except when such participation is contraindicated for medical reasons.
  • To provide patients with the knowledge of any fee for service. 
  • To allow patients the right to refuse to participate in experimental research.
  • To provide patients with a method for expressing their concerns and suggestions.
  • To allow patients the right to change clinicians should they so desire.
  • To inform patients of their obligation to provide the full and accurate information needed by health center personnel to assure their proper evaluation and care.
  • To encourage patients to interact effectively with the health care provider by asking questions concerning the diagnosis and treatment of the condition and expressing appropriate concerns about the recommended treatment.

RESPONSIBILITY AND AUTHORITY

The Director is primarily responsible for this policy. The Director shall delegate the responsibility for implementation of the policy to the entire staff of the Health Center.

PATIENT RIGHTS

  • To be provided with courteous, considerate care, as well as being treated with respect to privacy and confidentiality with regard to treatment and medical records.
  • To review medical records in the company of a professional, but they remain the property of the Alexander Health Center .
  • To be informed of the effectiveness of treatment, and to know of possible risks, side effects or alternate methods of treatment.
  • To have the choice of a clinician and to change clinicians if desired. 
  • To know who is treating you.
  • To refuse treatment, or to ask for a second opinion, or an alternative course of treatment, and to be informed of the medical consequences of your actions.
  • To be informed of personal responsibilities involved in seeking medical treatment and maintaining health and well-being after treatment.
  • To bring any dissatisfaction to the attention of the attending health professional or Health Center Director.

PATIENT RESPONSIBILITIES 

  • To present accurate identifying information.
  • To present details of illness or complaint in a direct and straightforward manner.
  • To cooperate responsibly with all persons involved in the health care process.
  • To keep appointments and be on time.
  • To cancel appointments only when absolutely necessary, and far enough in advance so that other patients might utilize that time.
  • To comply with the treatment plan provided by the health professional.
  • To ask for clarification whenever information or instructions are not understood.
  • To provide both positive and negative feedback to the health professional responsible for care.
