Oklahoma State Law requires that all students who attend Oklahoma colleges and universities must provide proof of immunization for certain diseases. Medical, religious, and personal exemptions are allowed by law and such requests must be made in writing using The University of Tulsa Certificate of Exemption form. We are pleased to announce the new service of an online Patient Portal. Required patient forms, forms related to your upcoming appointment, messages from/to your provider, lab results, and educational materials are all available on the patient portal. You can also access, input, upload and print your required immunization records.
Just click on the AHC Logo above and use your TU user name and password to get started using this exciting new service.
Oklahoma Law requires that first time enrollees, who reside in on-campus student housing, be vaccinated against meningococcal disease UNLESS:
- the individual signs a written waiver that he/she has reviewed the information provided by TU regarding the meningitis immunization and has chosen not to be immunized, or
- in the case of a minor, the individual's parent or guardian signs such a written waiver.
Once you input your immunization information and upload your immunization information, it will be verified by an AHC staff member and your student account will be noted as compliant.
FAILURE TO COMPLY WITH THESE REQUIREMENTS WILL RESULT IN A HOLD BEING PLACED ON FUTURE ENROLLMENT
The meningitis and hepatitis B vaccinations are available at the Alexander Health Center on campus. The MMR vaccination is available at the Tulsa Health Department. They can be reached at (918) 582-9355.
